Indications/Uses
Adjunctive therapy for partial seizures w/ & w/o secondary generalization in adults & childn ≥3 yr. Neuropathic pain including diabetic neuropathy, post-herpetic & trigeminal neuralgia in adults ≥18 yr.
Dosage/Direction for Use
Epilepsy Adult & ped >12 yr 900-3,600 mg daily, may be initiated by 300 mg tds on day 1, or by titrating dose as 300 mg once daily on day 1, 300 mg bd on day 2, 300 mg tds on day 3. Dose may be increased in 3 equally divided doses up to max: 3,600 mg daily. Ped 3-12 yr Initially 10-15 mg/kg daily in equally divided doses tds, ≥5 yr Effective dose: 25-35 mg/kg daily in equally divided doses tds, 3 to <5 yr 40 mg/kg daily in equally divided doses tds. Max time interval between doses: Not to exceed 12 hr. Neuropathic pain Adult Initially 900 mg daily in 3 equally divided doses, may be initiated by titrating dose as 300 mg once daily on day 1, 300 mg bd on day 2, 300 mg tds on day 3. Max dose: 3,600 mg daily. CrCl ≥80 mL/min 900-3,600 mg daily, 50-79 mL/min 600-1,800 mg daily, 30-49 mL/min 300-900 mg daily, 15-29 mL/min 150-600 mg daily, <15 mL/min 150-300 mg daily. Hemodialysis Loading dose: 300-400 mg, then 200-300 mg following each 4-hr hemodialysis.
Administration
May be taken with or without food.
Contraindications
Special Precautions
Hypersensitivity eg, fever or lymphadenopathy. Drug rash w/ eosinophilia & systemic symptoms. Not effective for absence seizures. Increased risk of suicidal thoughts or behaviors. Increased seizure frequency or precipitate status epilepticus. Monitor signs of CNS depression eg, somnolence; suicidality, worsening depression &/or unusual behavioral or mood changes eg, anxiety, agitation, hostility, mania & hypomania. Avoid abrupt discontinuation. Concomitant use w/ morphine. Renal impairment or hemodialysis. Ped 3-12 yr: Neuropsychiatric adverse events including emotional lability, hostility, thought disorder & hyperkinesia.
Adverse Reactions
Somnolence, dizziness, ataxia, fatigue, dysphagia.
Drug Interactions
Antacids containing Al w/ Mg, morphine, cimetidine, ginkgo, evening primrose, ketorolac, hydrocodone.
